About You — The Testator
Your personal details as they will appear in the Will. Please use your full legal name exactly as it appears on your Aadhaar / PAN card.
Family & Beneficiaries
List all persons you wish to provide for in your Will. You can add more beneficiaries. Include full legal names and their relationship to you.

List all assets you wish to cover in the Will. Be as specific as possible — property by survey/flat number, bank accounts by account number, investments by folio number. You can add more entries.

Distribution — How to Divide Your Estate
Tell us how you want specific assets distributed. You can give general instructions (e.g. "equal shares among my children") or specific bequests (e.g. "the Dwarka flat to my son"). Include a residuary clause for anything not specifically mentioned.
Executor & Witnesses
The executor is the person who will carry out your Will after your death. Choose someone reliable, ideally younger than you and not a beneficiary. You will need two witnesses at the time of signing — they must not be beneficiaries.

Aadhaar card (testator)Both sides — for identity and address verification in the Will.
PAN card (testator)Required if any bequests include financial assets or property valued above ₹50,000.
Title documents for each propertySale deed, gift deed, or inheritance document — to confirm ownership and description. Registry papers if available.
Aadhaar / identity proof for each major beneficiaryHelps ensure the Will correctly identifies beneficiaries and avoids disputes later.
Existing Will (if any) to be revokedIf you have a prior Will, please send a copy so we can include a clear revocation clause.
Recent photograph (testator)Passport-size photo — affixed to the Will for identification.
What happens next: Once this form is submitted and documents received, a draft Will is prepared and shared for your review. You will have the opportunity to make changes before execution. Execution requires your signature and two witnesses in the presence of the chamber — this can be arranged at a time convenient to you.
